Due to COVID-19, homebound seniors (65 and older) could experience more falls due to less physical activity as most continue to shelter in place. As a result, health care professionals might experience an increase in patient admission due to more non-fatal falls than previously experienced.
Falls can have long-term, life altering effects, which can be addressed in the hospital setting using validated screening tools and patient empowerment techniques for improved outcomes.

Using these techniques can result in less falls and a safer community.

In this webinar, world renowned fall expert, Tiffany Shubert, Ph.D./PT, Senior Product Manager of Therapy for Relias, will present the science behind falls and interventions that you can implement into your profession while Stephanie Wierzbicka, Manager, Strategic Health Care Programs, for in-home care provider ComForCare Franchise Systems, LLC, will present the role of the provider and the impact of this approach in the home environment.

During this webinar attendees will learn how to:

  • Describe risk factors that lead to a hospital admission or readmission due to a fall in older adults
  • Describe evidence-based interventions to minimize risk of a fall post-discharge from hospital to home
  • Identify community-based resources available to maximize function and minimize fall risk
  • Describe the role of every member of the care team, from patient, family member, clinician, and in-home worker in fall risk management
